It can be for several reasons, but here are 2 of the more common reasons:
1) Your club face is opening up during your swing. Try to roll your wrists slightly as you come through the ball to square up the face of the club to the ball.
2) You’re swinging across the ball. That is, your swing isn’t making a straight line as you hit through the ball. Your arms are coming closer to your body on your follow-through than during your backswing. Try extending your arms or ‘reaching out’ as you swing through to avoid this.
