Because you do not have a grooved swing,a definite swing plane or a proper tempo. If you enjoy the game, take a series of lessons from your area's best teaching pro. The cost will be well worth the results and subsequent enjoyment you will get from your improved ability.

Well I think you have hit upon the one thing that separates those of us that play golf for fun and those that play for a pay check. CONSISTENCY....I have attended a few PGA events and that is the one thing that really amazes me. Not that Tiger can hit a 310 yd drive because I can....But they (pros) do it 99% of the time !
Hence is why it is soooo important to develop a swing that is natural for YOU. If and when you find this natural swing, and teak it here and there a bit, you will be able to reproduce this swing over and over again, as opposed to a more methodically mechanical text book swing.
If you watch Fred Couple's swing....natural and with ease....His tempo his perfect....When you have found this swing, then you will start to become more and more trusting of it and you will begin to swing with better tempo and thus looking like being effortless. If you try and kill the ball from the start you will have a tendancy to hit with an open face and slice off to the right. At address make sure you hands are slightly ahead of the ball and the ball is positioned just off your front foot's instep. Now as you begin you back swing, start to motion with your front shoulder and not your hands, bringing the club back slowly. It makes no sence to hurry your back swing because the club has to come to a complete stop and reverse direction anyway. A quick back swing will also promote pulling you off your position. Try to keep your front wrist from cupping. If you are slicing, I am almost sure your wrist is cupped. Bring the club back on a more flatter swing plane.
Now at the top of your backswing and start the downswing, again start the turn with your shoulders and not your hands...Pull with your left side and don't push with your right. Pull as if you are going to try and hit the inside/back portion of the ball. Make the biggest arc possible moving your weight to your front foot. You will know of you are doing this in your follow thru. You are either standing on your front foot or not.
Pay close attention to the flight of the ball as it starts out. This will tell you a lot. If it starts out bending hard right....SLICE....if it starts out straight and begins bending to the right..a CUT FADE...If you begin to flatten out your swing keeping that front wrist from cupping and really pulling with your left side...you should get a Right to Left draw...draws are great off the tees because it is a over the top spin on the ball. Therefore when it hits the ground it will go further down range ans stay in the fairway where as a Fade is more of a backspin. When this hits the ground it typically exits stage right. TEMPO, TEMPO TEMPO.
Go to the range and for the first ten balls or so, try to go through your entire swing motion but with little or no effort. Do Not try and crush it.
You may be amazed at how far you hit it leting the club work and not you. If you watch the Champions Tour, almost every pro hits with the same effortless Tempo....and hit 270-280 yds...Couples looks like it is effortless but I promise you in his mind he is trying to crush it...but staying within his TEMPO....Hope this helps !

A philosopher once said that the only consistency of man was his inconsistencies. Even the Tour Pros have inconsistent days and they are about as good as it gets at this game of golf. The only advice I can give you is that on those days when the slice comes in......take everything back to basics...check your grip...your stance...your alignment...if that does not solve it...do what the Pros do...you just grind it out and come back tomorrow.

It happens to everyone. It can be more severe for some people though. This is probably caused by not having the same swing every time. When your at the range your going to want to practice keeping your swing on plane when you practice.

A duck hook tends to hook immediately off of the clubface.

Based on your description, I would guess that you are setting up with too much weight on your heels: http://www.golfmedic.net/archive/weight-on-heels/

Check to make sure the ball is not placed too far back in your stance. Another thing you might want to check is to make sure the "V" formed by your thumbs and forefingers are both pointing towards your trailing shoulder. This is a mistake that I make at the beginning of each season and end up hooking the ball.

This is a common problem. My guess is that you have an upright swing plane. When you swing back your whole body lifts up and you move your head to the left. Try leaning your weight to the right before you hit the ball. Think about taking a flat swing more like baseball or softball. Remember you don't have to take it back really far to hit it really far. It's all about control and swing speed.
